A Clinical Study on the Traumatic Rupture of the Spleen


Abstract
This paper is a retrospective study on 31 cases of traumatic rupture of the spleen, which were treated at Department of Surgery, Chungnam National University Hospital during the past 5 years and 2 months from May 1985 to July 1990.
@ES The following results were obtained:
1) As to the age distribution, the peak age incidence was in third decade(26%), followed by in 2nd and 5th decade.
2) Males were predominated over females by 9.3 : 1.
3) The causes of the splenic injuries were nonpenetrating injuries(93.5%) penetrating injuries (6.5%), and the most frequent cause was traffic accidents(64.5%), followed by fall down injuries (22.5%).
4) Among symptoms and signs, abdominal pain(90.3%) was most frequent, followed by generalized abdominal tenderness(80.6%), rebound tenderness(74.2%), and abdominal distension(58%).
5) Paracentesis was positive in 26 cases(89.6%).
6) The average time interval between injury and operation was 7 hours.
7) The amount of intraabdominal blood loss was ranged from 250 ml to 4000 ml.
The average amount was 2000 ml.
8) The average amount of transfused blood was 1600 ml.
9) The associated injuries were noted in 22 cases(71%), and the most frequent injuries were rib fracture(11 cases) and hemopneumothorax(10 cases).
10) The most common type of splenic injury was Type III(51.5%), followed by Type II(32.5%).
11) As to the management, splenectomy was performed in 24 cases(77.5%) and 7 cases(22.5%) were managed by splenorrhaphy.
12) The incidence of postoperative complications was 32.5% and the most common complication was pulmonary complications(19%).
13) Mortality rates was 9.5%(3 cases).
